Case Summary:
Mandible Reconstruction and Fibula Free Flap Guides
Mandible Reconstruction and Fibula Free Flap Guides
Patient Diagnosis:
This patient required mandibular reconstruction.[needs work- for?-
Planned Procedure/Approach:
The surgeon requested assistance in planning the resection from LL4 to the mandible border as well as the harvest of a segment of the right fibula to reconstruct . They planned for a fibula free flap transplant with an anterior vessel connection (distal end of the fibula facing posteriorly).
The mandible would be repositioned to provide a better occlusion, and the left ramus body slightly rotated.
What Insight Surgery provided:
- Patient-specific 3D virtual and 3D printed anatomical models
- Surgical Guides – Mandible guides designed with cutting flanges to accommodate any blade, as well as pre-drilling holes to allow for guide fixation and subsequent fixation of the custom titanium plate for the reconstruction. Datum points were also included on the anterior guide to directed towards three teeth while those on the posterior guide where aimed at bony landmarks to aid with guide location intraoperatively.
- Surgical Guide – Right fibula guide for a distal cut
Outcome/Benefits:
- The surgeon appreciated the accuracy and ease of guide location provided through the fixation and datum points.
- The provision of a custom mandible plate was factored into Insight Surgery’s guide design with pre-drilled holes, allowing for a reconstruction tailored to the patient and leading to a outcome in terms of reconstruction.
